Anne Gavic Award

This award is intended to recognize an individual who has made significant contributions to ISCHR and to the profession of Cardiac and Pulmonary Rehabilitation. 

Criteria:

Applications are open to all cardiac and pulmonary rehabilitation professionals from Illinois who are a current member of ISCHR/AACVPR and have attended an ISCHR conference.  

Award applications will be reviewed and a winner will be selected by the ISCHR awards task force and approved by the ISCHR Board of Directors.  

The recipient of the award will be given a year Affiliate ISCHR/AACVPR membership and registration to the upcoming year's Annual AACVPR National Conference.

Deadline for submissions: February 28th, 2026 

The winner will be announced at the ISCHR Annual Conference on March 14th, 2026

Nomination Steps:

1.To nominate a colleague or yourself:

  • Complete the nominees contact information below. 
  • Provide a brief statement of support for the nominee
  •  If you are nominating yourself-please request a letter of support from a colleague 

2. The nominee will be contacted to complete the full nomination form and provide additional information including:

  • brief personal statement that highlights

For example:

  • Your career accomplishments working in Cardiac and/or Pulmonary Rehabilitation 
  • Your contributions to the Cardiac and/or Pulmonary Rehabilitation profession
  • What you think are the benefits of being an ISCHR member
  • Anticipated benefits from attending the AACVPR Annual Meeting
  • Your resume/Curriculum Vitae in PDF format
